Pharmacist Job Responsibilities for Medication Therapy Management (MTM)

Medication Therapy Management (MTM) Pharmacists at Payless Staffing and Consultants Inc.

Key Responsibilities:

  • Conduct comprehensive medication reviews for patients to identify potential drug therapy problems, optimize drug therapy, and improve health outcomes.
  • Collaborate and consult with healthcare providers to develop and implement medication therapy management plans that address the patient’s needs, preferences, and goals of therapy.
  • Provide patient education on medication use, potential side effects, and ways to manage and prevent potential adverse effects.
  • Monitor medication therapy outcomes and adjust drug therapy as necessary to achieve optimal clinical outcomes and minimize drug-related problems.
  • Document medication therapy management activities in patient records and communicate with healthcare providers to ensure continuity of care.
  • Participate in quality improvement initiatives to enhance the safety, effectiveness, and efficiency of medication use.
  • Stay up-to-date on emerging trends and best practices in medication therapy management and incorporate them into practice.

Qualifications:

  • Doctor of Pharmacy degree from an accredited school of pharmacy or qualified foreign graduate.
  • Strong clinical knowledge and expertise in medication therapy management, drug interactions, and adverse drug events.
  • Excellent communication skills and ability to work collaboratively with healthcare providers and patients.
  • Detail-oriented, organized, and able to manage multiple tasks and priorities.
  • Proficient in using electronic health records (EHRs) and other healthcare technology.

Management of Controlled Substances

  • Adhere to regulations outlined in the Controlled Substance Act (CSA) to track and secure controlled substances effectively.
  • Maintain control measures to prevent the diversion and overuse of controlled substances.
  • Provide training for accurate record-keeping and reporting related to controlled substances.

Management of Hazardous Waste

  • Provide consultation for proper handling and destruction of pharmaceutical waste.
  • Follow appropriate protocols for storing, handling, and disposing of hazardous waste and appropriate handling of outdated pharmaceutical waste in a safe and environmentally responsible manner.

In carrying out these responsibilities, a pharmacy staff will need to be familiar with the guidelines and requirements set forth by the board of pharmacy and DEA in each respective area. By adhering to these regulations, you will contribute to maintaining the integrity of the drug supply chain, preventing substance abuse, and ensuring the safe management of pharmaceutical waste.
